The crash is repeatable. However, it only happened after a few uses
of LyX. That is, I was able to open a few files, then all of of a
sudden, it started crashing every time.

Anything else I can try to narrow down the problem?
Dumb question: does this happen for any file or just a specific one, a set
of files, or files in a specific directory (with spaces in the path name,
say).

Hmm, I did not try a comprehensive test, But it happened for any file from a
limited group of directories that have no spaces in the path name).
I seem to remember some problems like this from a while ago that ended
up having to do with hidden directories that were triggering bugs in Qt
itself.
