Reconfiguring does not resolve the problem. I get the same symptoms.
I gave the wrong source of lyx file below.
The lyx file that is giving me the problem is the User's Guide fetched
via the Help tab, not the web site indicated below.
So the bug still exists. Maybe the lyx file that comes with the program
is faulty.
As workaround I'm using the merged 1.6.4/Manuals.pdf file from
http://wiki.lyx.org/LyX/Manuals

So your LaTeX installation misses the package "subfig". To fix this
- open an Internet connection
- start LyX by double-clicking on the file "lyx.exe"

MiKTeX (the LaTeX engine) will now install all LaTeX-packages needed
by LyX. You can see the progress in the console window that was opened
together with LyX.
