If you look in the View Source window you'll see that you have got
\noun{Test\index{Test@\noun{Test}}}
whereas what you want is
\noun{Test}\index{Test@\noun{Test}}
(Turn the nouning off before clicking on the Insert index entry button,
then format the text inside the index inset.)
