Thanks much for your post as it lead me to the answer. Since I only had one
visible occurrence of the contact in question, the solution wasn't clear to
me or the person who helped trouble shoot, so I thought I'd augment John's
explanation a bit.
I am syncing contacts with iCloud, hence have the option unchecked in
iTunes. If I use the edit button and associate a contact with an available
m4r file, it will appear to be correct, except that the default tone
continues to sound. If I go past the "linked cards" heading and select the
"iCloud, contact name" button, it continues to display either the default
ringtone, or a different one that was associated previously. If I double
tap the "unified, contact name" button it correctly indicates that multiple
ring tones are associated. Lesson learned is that the decision of which
ring/text tone sounds is governed by the "unified" setting, which apparently
resolves all disparities in favor of the value specified in iCloud. Changes
to the local version are *NOT* propagated to the cloud; at least not
ringtone preferences anyway. They will be updated on both the iCloud and
local version of a contact if and only if the changes are made via the
dialog resulting from selecting the "unified, contact name" button.

I have no idea if this is the problem you are having, but I was having a
similar issue with several of my own created ringtones that I would
associate with specific contacts. When the individual contact would call,
the default ringtone would sound, and it drove me crazy for a while.
What I discovered was that I actually had multiple entries listed for
the same contact. I suspect this had to do with syncing with iCloud, but
when I removed other duplicates and then made sure the proper ringtone was
associated with the correct contact, it worked fine. BTW, the same thing
was happening with text tones too.

observable on several files. I verified that there is nothing in the
app switcher because I've seen random weirdness disappear in the past by
clearing it.
My questions now are whether anyone can suggest trouble shooting steps
that would help isolate the problem. Since this is reproducible on two
phones, I assume others could observe my results. Since the results do
not appear to be consistent from one phone to another of the same model
running identical versions of IOS5, the trigger is obviously eluding me
so far.
