The options that show up in a context menu have to be coded that way, I think.  
I have a vague recollection from when iTunes was transitioning in accessibility 
that there was an instance where the options that appeared with VO-Shift-M were 
different from what you got when you used Control-Click -- in the sense that 
they had added a feature under VoiceOver.  

I distinguish between "software clicks" (VO-Shift-Space) and "hardware clicks" 
with Mouse or Trackpad.  VO-Space is actually the command to "perform the 
default action" -- which could include clicking on a link.  In applications 
with transitioning accessibility, I may end up having to perform "hardware 
clicks".  By the way, pressing the "5" key on a Numeric Keypad with NumPad 
Commander turned on works the same way as a hardware click.  So does turning 
Mouse Keys on and pressing the "i" key.  This would explain why adding 
"Command" to these combinations doesn't work.  I can do a Command-Click by 
turning on Mouse Keys (set to turn on by pressing the Option key 5 times on my 
machine), and pressing Command-i, where "i" is the key used to "click" with 
Mouse Keys turned on -- it's in the position of the "5" key where the embedded 
numeric keypad used to be.  However, I think I recall reading some posts that 
clicking this way didn't always seem to work under Lion.  I'm afraid I can't
  remember more about this -- the discussion was on another list.

It sounds as though there is a focus issue with Safari.  I'm still using Safari 
5.05 and Snow Leopard. Sorry not to have more suggestions.
