You'll find that the volume will randomly drop -- some times the change is probably attributable to an inadvertent holding of the volume down key. Still, I suspect there may be a bug somewhere. For example, when I pull my phone out of a dock, the speaker volume may be all the way down.
If the remedy you discovered does not work, and no sound other than ringtones is emitted from your speaker, there may be a dust particle caught in the headphone jack. This would cause the microswitch to remain open and consequently your hardware behaves as though headphones are connected regardless. When this occurs, you can usually dislodge the obstruction by inserting and withdrawing headphones into your jack about ten times quickly. A vacuum or canned air would also do the trick. I have not yet experienced this problem on my 4S, but assume the hardware is unchanged and hence that possibility still exists, so its something to bare in mind. Best regards.
