The solution I think you're looking for is called macSpeech. It is made by 
Nuance, and you can find the Homepage here:

Please bare in mind, that my comments are based on past enquiries to Nuance. 
That being said, I do not know if accessibility has improved recently.. The 
last time I saw an in-depth discussion on-list about this, I seem to remember 
that the initial training for the app was inaccessible. This training is 
important, as it allows MacSpeech to become accustomed to your voice, although 
it may be possible to bypass this and train as you go.
