I'll second or third that. The practice of pressing Command+F5 twice to restart 
VO has become a standard part of my web-surfing routine. Sometimes this does 
not clear up the issue. In those cases I usually assume there is a memory leak 
or some other issue happening that is going to crash the system anyway, so I'll 
save any open work and restart the computer just to be safe.
