Hello. you can start voice over at the log in prompt and have it read as normal.

I would not disable the guest account as it can be a good test environment, how 
ever if you want to go do you can do this.

1. launch system preferences.
2. go to users and groups
3. go to the final entry in the table. You may need to unlock your preferences. 
4. Uncheck guest account and close the window.
