I've successfully used regular headphones normally used for stereo and the
built-in iMac microphone to communicate with a friend in Spain. It's been a
while since we talked so I can't remember the settings but it does work.
Skype allows for instant messaging so play with the settings as you're
trying to connect to a patient contact.
