You have a classic ethernet combo network card there. 
The coax connector (aka BNC) is for thin ethernet, the one that looks like a
large phone plug is an RJ45 connector - the standard ethernet connector of
today, finally what appears to be a video connector is an AUI port. This is for
thick ethernet adapters, aka vampire taps. 
Don't worry about mistaking it for a video port, I once came across a PC owner
with a joystick plugged into one of these ports complaining that it didn't work!
