It depends on the way you set up iSync.

In the iSync window, if you click on one of the PDA icons you will get 
a drop-down window. One of the options there (under Calendars) is 
"Ignore Palm events older than <1 week> <1 month> <3 months> <6 months> 
<1 year> <2 years>". If you have that box checked, and you wait longer 
than that amount of time before you sync, synchronization won't pick up 
the old changes. (This option is to make syncing more efficient, so 
that all your computers aren't comparing events from the beginning of 
time every single time you sync.)

On the same iSync drop-down, you can tell it (under "Contacts") to 
synchronize "All contacts" or only certain groups. "All contacts" would 
ensure that every entry is updated to the newest information with each 
sync.

(If you want to watch iSync paralyze your computer for an hour, create 
an event and set it to repeat weekly with no ending date. Then a year 
or so later, delete "this event and all future occurrences". There are 
a lot of weeks between now and the end of Time According To Apple, and 
it takes a long time to erase them all.)
