There are many people saying that Mail doesn't sync properly with Exchange servers. (And Gmail.) I haven't seen any problems with iCloud or more or less standard IMAP servers. I also think Mail gets confused with large mailboxes containing thousands of messages. I’m working on solving this problem by archiving really old mail. Here's an article listing alternatives.
