If you can't find this info anywhere else, there are a ton of USR 56K modems
for sale on eBay.  You could look at the listings with pictures, find a few
that were the same modem as yours and weren't still sealed in the box and
looked like somewhat friendly sellers, and use the "ask seller a question"
feature to send them a message and ask if they would be so kind as to maybe
if they got the time check their modem's adapter and let you know what the
volts, amps, AC or DC type, and polarity (if DC) listed on it might be.
