Off the top of my head, have you tried -f (force)? What is the output of sudo port -d uninstall transmission?
There are a few other ways to deal with this, but I think the above would be a good start to see what is going on.

I used port to install transmission and then have been updating from within Transmission since then. That means that the filesystem no longer has what port thinks should be there. I can't get it to uninstall because of a `realpath` error.
