Why does this file exist already?

You can force the activation:

sudo port -f activate freetype

But this file should not already have existed. The fact that it does suggests 
that you previously had MacPorts installed, and somehow uninstalled it 
incompletely. You may want to uninstall it correctly now, following the 
instructions in the Guide, and then reinstall the ports need, because it might 
not just be parts of the old freetype that are still on your hard drive, and 
they might cause problems later.
