A simple solution is if you want some features, send a note to 
accessibil...@apple.com 
  and it may find its way to the right folks. This is not open source,  
so obviously it can't be modified in the manner you would like.  
However, Apple is open to feedback and welcomes such from users. A lot  
of what you find in VoiceOver has been recommendations/suggestions  
from users.
