You know I have not tried this in a good while, but one thing that may  
or may not have a role in this and might explain why it works with  
iTunes and not VoiceOver is this. Music is a constant stream of audio  
where VoiceOver is not. Now with this said and based only on my  
experience, I think what happen in my case is the audio channel on the  
Mac closes when there is no sound, so if VO is not speaking or there  
is no music, the channel is closed and the headset would go to sleep.  
Now whether this is your issue I don't know, but something to  
consider. As far as why something is spoken twice is odd and I don't  
recall encountering that situation.
