I had to do this recently, and it's a bit of a pain in the ass. First, you navigate the sidebar to get to the Skype home. Interact with the HTML area and find the image with your Skype name and VO+space. Find the button that says "Profile picture" and VO+space. Navigate down the "Field" table (you'll have to interact with it) to "Other".
Then, you have to get to the file selection window. VO+F2 twice and select the "Open from Skype" window by pressing return on it. Find your photo as usual and press the "Open" button.
