Keep in mind that speech can be muted even though VoiceOver may be active. I guess this is what you are implying, but I know that on the iPhone a specific gesture can mute speech despite the presence of VoiceOver. I would imagine other iDevices would have the same gesture.

I have an ipad where I can toggle on voiceover using siri, but VoiceOver doesn't speak.
I think the volume is down on the machine.
Besides the buttons is there a way to turn up VoiceOver speech?
