Make sure the camera is above the document or object, about 6-8 inches
(15-20 CM), or about the distance from your elbow to your hand. So, put
your elbow on a flat surface, and hold the phone as noted. Start by
putting the phone flat against the object if possible, then raising it
up to the position noted above. If you take a picture at that point, you
may have good results, though it's kind of an art and does necessitate a
bit of practice.
