First, yes, Bria can be running in the background and you can receive calls.  
It will pop up like google voice or other apps when you receive a call.  You 
can accept or block and so forth with the commands available in the popup.

Keepalives are used to keep a path open used in combination with SIP arg and 
NAT.  Pretty much your defaults should do the trick.
