Jonathon, if the dog is well trained it should be able to find the entrances 
you require and the same is true with escalators. Shoes should not be mandatory 
when using an escalator.  That is, if, the dog is well trained.

I have an app on my phone called My Way. It is a bit clunky to use maybe 
because I have the Lite / free version.

 

You can ask it to record a route. Then every time you either shake the phone or 
make a significant turn it will record that spot in a list of waypoints. You 
can then follow that path forward or backwards.

 

I will be looking for the full version later this week since I am about 2 
months out from getting a new guide dog and I am not sure how well I'll be able 
to find the dozen or so subway entrances I use on a regular basis. I usually 
don't need the turn by turn information but just how far down the block a set 
of stairs or escalator is. Though considering thatGDB now expects you to put 
shoes on the dog before using an escalator, I might be searching out all the 
elevators.
