Are you able to successfully download apps in the Mac App store? If I remember correctly, then the synthesizer downloads are just asking the App store to download appropriate materials.
Personally for me, I went into High Sierra Voices, selected and deselected a few check boxes items that were newly selected said downloading 0% and didn’t not change. I then close the voice selector read a few e-mails and then returned to VoiceOver Utility. At that point I was able to use the newly selected voices. I don’t believe there is a standard library CLI for managing voices much like the diskutil can manage disk drive administration, and in any case, the underlying services would still need to be the same.
