Well, if you have been using Mac OS for several years and mostly doing 
upgrades, it might be time to reset your keychain. Note, any passwords saved in 
your keychain will not be directly available to you if you do this process. If 
you keep getting prompted to enter mail passwords every time you  launch mail 
then there is a good chance you should do this. Open the Keychain Access 
program found in the Utilities directory, and then open its preferences. In 
there there is a button to reset your keychains when you do this any passwords 
saved in your local keychain that are not stored in the iCloud keychain will be 
unavailable but not totally lost. This is because Keychain access will first 
save a copy of the keychain. So you could always add the saved keychain into 
keychain access and copy a password saved in the saved keychain back into your 
current keychain.
