This has not been fully tested, but should work…

1. Open Script Editor from the utilities 
2. Paste the following line into the text edit field 
tell application "System Events" to start current screen saver

3. type command "R" and your computer should immediately start screen saver 
including prompting for password if so configured.
4. Get back into script editor and save the file.
5. Open VoiceOver Utility  and move to the commanders area.
6. Pick a commander and the keys you want to use and then pick run AppleScript.

Note: It might be that the first time you  request that VoiceOver run the 
screen saver, that you will get a scrod needs permissions to run a command. You 
would need to allow this.
