I’m partial to Emacs, with Emacspeak of course, and Org-mode. Just write your 
file, 
** Make headings

** make a list like this:
- Unordered list item
1. Ordered item
- Italics is made like /this/.
- Bold is made like *this*.

You can also just write Latex in it, and it’ll come out exported as Latex would 
be in that format.
After you’re done, you can just export the file as HTML, ODT, Plain Text, or 
Markdown. Then, if you need to, you can use Pandoc to convert the ODT file to 
Word format or RTF, I assume Pandoc should support RTF.

Using Org-mode, or even Markdown, allows you to really control the formatting 
of a document, and Emacspeak helps by changing pitch and entonation to call 
attention to formatting, along with the symbols you use for denoting formatting.
