I don't think there is a single button that will hide all those things. I think 
you have to  look at each request and choose the button that says "don't allow 
this application. It took me a while but I finally got rid of all mine. Now I 
just get one whenever they get a new game or something.  Using the 
m.facebook.com  site helps too.
