Hi Dean and Gigi,

Actually, couldn't your friend try using the three-finger double tap on the 
TrackPad to un-mute VoiceOver?   That was my first thought, but I assumed that 
you had tried to have him do that first. Gigi is correct, it helps to keep your 
fingers slightly curved as well as separated when you do the double tap.  The 
other possibility for new users is that they have turned TrackPad Commander 
off, if they've also managed to mute VoiceOver.  In that case, hold down the 
Control and Option keys, and make a rotor gesture in the clockwise direction 
with two fingers on the trackpad, then try the two-finger triple tap again.

Alternatively, could this be one of the situations where VoiceOver is busy? 
This is more likely to happen in Safari, or perhaps in Mail if lots of windows 
are open, and occasionally in the iTunes Store.  What happens if he uses 
Command-Tab to switch to another application, and then toggles VoiceOver off 
and on again with Command-F5?  That should bring back VoiceOver even if it has 
been accidentally muted.

Just trying to think out loud about possibilities.
