Basically the sources for the icons are different. The application manager shows the icon it finds in the control file of the debian folder, while the icon in the task launcher is based on the one in the icons folder. But can you see the blue square as the default icon in the task launcher? Then your problem is probably related to a bug [1] in the system. Does your icon shows up after a restart?
