But if the Approved line is a header, will mailman strip it out so others don't see it?
It will in plain text, but if the list allows HTML and the post contains HTML the approved header will make it through.
Will either method work? Is there a configuration option that needs to be set?

Mailman is already set up for this - it's a matter of getting your mail program to do the rest. Try making it the first line in the body of your message. So start with:
Approved: password
Where password is the admin or moderator password.
