Debian Exim (pkg-exim4), can, almost, be made to work like Proper Exim
should, with a monolithic exim4.conf file, and a few tweaks to
/etc/{default,init.d}/exim4.
I can't remember where my patches are, but they're not in The Usual
Place, annoyingly.
