They're almost certainly forwards. A fair number of people have permanent addresses through a university or professional association (I'm u...@computer.org) which is forwarded to whatever ISP mail they happen to be using at the moment. So the unsub comes from the ISP address. There's also mail where the incoming addresses and outgoing are in different subdomains, e.g. you send mail to rob...@bigcompany.com and the reply comes back from b...@hq.bigcompany.com. As Al said, you need to put a token in the unsub link or address that tells you what address the message was sent to so you know who to remove.
