Given some of the points in this thread, I'll expand a bit, hopefully
without sounding too self-promotional.
We're Canadian, our main data center where the IPs are is in Toronto
(Markham)
Our main netblocks are 64.68.192.0/20 with the VPS pool currently being
allocated out of 64.68.204.0/24 and 64.68.206.0/24 (if you wanted to
gauge the IP reps) - our ASN is AS16686. IPv6 supported (and functional).
Our SOP is to block outbound SMTP on new VPS on new accounts (I can't
remember the account age requirements, it could be 6 weeks) but anybody
here can just email me off list and we'll remove the restrictions.
I think it's important that independent and self-hosted mail operators
are able to continue - we see a lot of talk around how hard that's getting.
We have the occasional hiccup here (usually a compromised account on our
own hosted IMAP) - those are in separate blocks, but we run a tight ship
and have all kinds of early warning systems on the VPS instances.

Panix in NYC is a very old very well run ISP. You can do mail hosting but you
need to open a ticket to ask to get mail ports unblocked and explain why you
need it. panix.com
Also look at Tornado VPS tornadovps.com whosse motto is "We do not assume you are
stupid"
They allow sending mail by default, but I have never seen spam from them. They
mean what
they say. If you ask reasonable questions, you get reasonable answers. Dumb
questions,
not so much.
