Hi Brian, I found MsSqlSpatial good enough for one of our projects. We are using King.MsSqlSpatial FDO provider and we do MapGuide view of data and with AutoCAD editing of spatial data in SQL Sever using King.MsSqlSpatial. We used Fdo2Fdo to copy data from SHP to SqlServer.

PostGIS / PostgreSQL is an extremely capable and robust spatial database system. Its capabilities are similar to SQL Server 2005, apart from some of the higher-end enterprise features. The PostGIS provider is still in early days, but is settling down nicely. There are apparently .Net drivers available for PostgreSQL; I don't know if this means that it integrates into Visual Studio the same way that MS SQL does though.
