Are you running SQLExpress or full SQL Server?  If it is Express then the 
service name needs to SERVERNAME\SQLEXPRESS, if it is Full SQL then just the 
SERVERNAME...  The datastore field automatically populates after you hit the 
login button and successfully login.

I have tried all different combinations in the Service and Datastore fields
but keep getting the same error when I click test connection:

An exception occurred in FDO component.
RDBMS: No current open database.  

Any ideas?
