What probably happens is that your hole is very small. When this happens we may be tempted to decrease the filter but be careful the definition of the filter is that the hole will be considered if it is smaller than the filter. You must therefore increase the latter.
I also once had a problem with an extremely narrow hole (close to the
limit of precision) and I could not figure out why it was not considered.
Check if this is the case with your polylines. If not, send me your
table.
