I'm only aware of one way to set up styles in mapnik, which is to state the
pixels width(and I could be wrong in this)

<CssParameter name="stroke">white</CssParameter>
<CssParameter name="stroke-width">.5</CssParameter>
</LineSymbolizer>
I'm not sure exactly what is happening in the pdf output in relation to
sizing...
I think it is stuck at 96ppi

Good start.
What I'd like to see is a little description of how you describe the
size of things. The geo-data itself should get scaled appropriately but
how do you specify the width of lines, size of text, etc? If you specify
in pixels that's straightforward (but what is it for a scalable output,
like pdf?) if you specify in inches, points, or mm, then you need to
somewhere specify the PPI, or if it is indeed fixed at 96ppi, then you
need to scale stuff yourself. What are the options with mapnik?
