I got the hint on the OSM mailing list to change:

extent = -180,-90,180,89.99

Using these values, will will ran into problems with several
projections when rendering the complete planet. Better use this:

extent = -179.5,-89.5,179.5,89.5

This might also work:

extent = -179.9,-89.9,179.9,89.9

Googling for the problem i also saw that some people use an srs like:

'+proj=latlon +datum=WGS84'

You have to set this for the PSQL-layers.

Previously i used:

'+proj=merc +a=6378137 +b=6378137 +lat_ts=0.0 +lon_0=0.0 +x_0=0.0 +y_0=0 +k=1.0 +units=m +nadgri...@null +no_defs +over'

What is your target projection?

What else do i need to change to render maps?

You might also need to transform the shapefiles into your target
projection. Transforming them in realtime somehow never worked
here - and performance isn't thaaat good.
