(2) "/var/www/cgi-bin/mapserv.cgi -v" reports
including "INPUT=TIFF" and "INPUT=GDAL".]

Two possibilities come to mind:
1) The file is corrupt. Does it work outside MapServer?
2) MapServer was built without internal-tiff or GDAL support.
Does mapserv -v report INPUT=GDAL?
