Does that MAPFILE actually exist in that location on the server: map=/ms4w/apps/tutorial/htdocs/example1-1.map
If it does, then you may have a config/permissions problem on the (root of the) server. Mac OS has it’s own web service for example and it sometimes takes a little fiddling to get add-on working.
