Oops.... have to correct myself here: metres/pixel IS in relation to real-world coordinates (not paperspace). Forgot about that. So that has the scale integrated into it. The dpi resolution from scanning is not in relation to real-world coordinates, but paperspace.

Sorry, sleep deprivation and geo-calculations don't combine well.....
