The extent that you set in your map file should be set to the full extent of all of the data that you will display on the map. If you look at the URL that the template page creates, it is likely passing a different bounding box for the extent of the data that you want to see for a particular image.
I would simplify your map file even more: 1. Comment out your projection blocks. If you are only showing data from one projection, you don't need to include projection blocks. 2. set your layer STATUS to DEFAULT for now. This will make it draw regardless of whether you explicitly ask for the layer in the URL GET or POST. 3. Comment out your expression. If your expression isn't quite right, you may be eliminating all of the features in the data set. It would be useful to have you also include the URL that the page is using to call MapServer.
