I've been reading thru the TiffanyOS document, and IMHO, Section 22 may have the 
information you need to resolve your problem.

It refers to *.mil domains, but the symptoms sound similar.

    Reset lpc-restart=1 and ipcp-restart=1 in your /etc/ppp/options file.
