It's a bit subtle.  :-)

The R# integration (assuming you're not using R# 5.0 beta) just takes the
form of little annotations next to your tests so you can run them there.

The VS integration adds support for various Gallio-supported test framework,
such as MbUnit, to Visual Studio's own test runner.  Basically you just need
to create a new VS Test Project, add references to MbUnit (or another
framework) and write your tests.  Use the standard VS Test View to run
tests.
