Not a feature request, and not sure if this has been fixed in the latest version, but RTF attachments get treated as text and thus get corrupted. Would be nice to have that one sorted out ;-) I also remember sending a feature request to Oliver Wagner about the ARexx interface and possibly a C API for handling the database without MD2 itself. The reason was to be able to interface with things such as a Palm or other PDA for syncing mail between a device and MD2. So, from the top of my head, things like import mail(s), export mail(s), delete mail(s), export mail(s) truncated to a given size or with attachments stripped out, set flags like replied/forwarded, those sorts of things.
