There has been some discussion of digital clipping
and how the "over" indicator behaves.  While this
depends on the actual implementation, here are
the important issues:

On record, the "over" indicator should indicate when
the maximum sample value has been exceeded.

Since it is impossible to record a value greater than the
maximum sample value, there should never be any
true "over" during playback.  However, some equipment
infers that 3 contiguous samples of the maximum value
are almost always caused by an "over" during record.
That is, it infers that the middle sample of the 3 must
have been "over". 

That stated, it should be pointed out that consumer
equipment is often not this rigorous when it comes to
level indication.  However, I should also point out that
occasional "overs" are almost always inaudible.
