I guess you're talking about a stereo minijack female plug on
the card side, and a RCA (Cinch) female plug on the MD unit.

Do you know if that card has S/PDIF in capabilities? (then the card plug
should be stereo) Can you determine if the card plug is stereo or mono?

If...

is stereo: You only need a cable (pretty easy to find or build) with a
male stereo minijack on one side, and two female RCAs on the other. The
shortest, the better, you only need it fo "Y" purposes.

Right channel (Ring on the minijack, the Red RCA) channel uses to be for
Receive/Record, and Left (tip of the minijack, Black/White RCA) will be
for sending out of the card. Attach a 75 Ohm coaxial cable (is used a lot
for video) male RCA/male RCA wherever you need; if you want to send from
the card to the MD unit, attach it to the Black/White RCA, an then to the
MD.

is mono: You don't need a "Y" cable. Just make (or buy) a 75
Ohm coaxial cable with a male mono minijack plug on one side and a male
RCA on the other.

        Hope it helps,
