We have npr reports across med/surg, tele, mother/baby, etc that are
used in conjunction w/ the profile format.
The profile format contains the current ADS, orders in defined
categories, & the plan of care - this remains at the nursing station &
is used by all.

The npr is one page per patient & has a brief history, list of
procedures, risks (falls, skin, etc), pain & acceptable level, also last
documented assessments (CV, resp, skin, etc)
selected interventions code status w/ text, vs w/ freq, etc.

The nursing techs/assistants have a trimmed down version.

I don't think it is a step back at all - in fact it can be used
effectively to promote consistent safe communication with the bonus of
complying with the handoff standard.

All,

I am having issues with our nursing staff because they are not happy
with the current documentation that they use for reporting on patients
between shifts.  They originally had a manual process using a tri-fold
Cardex that they would write information on.  This then changed to a JOT
sheet printed from Meditech that listed the patients on the units and
their information from the system.  We now have print profiles set up in
Meditech and they were using a Cardex printed from a patient profile,
but now are not happy with that.

New issue is that they want me to create an NPR report to pull patient
information, but be similar to their original tri-fold Cardex they could
write on.  I feel we are taking a step backwards and need to utilize the
Print Patient Profile function to obtain all the orders in the system
and ensure the nurses on the last shift actually entered orders as
requested.  I'm not sure which way we should go with this request from
here.  What are other hospitals doing to handle reports between nurses
during shift change???
