Also, the vital sign documentation occurs on the same screen with queries that
only nurses are permitted to answer. It is not a CDS, so you can't put
attributes on the queries to prevent the CNA's from documenting on these
queries. This gives you two choices-give only the nurses access to this screen
and they will have to document all transfusion vitals or give the nurses and
CNA's access and tell the CNA's not to document on the nurse only queries.

The issues that we have submitted to Meditech to address are:

1) Method to require or ensure that nurses have reviewed any "Special
Instructions" attached to the blood product.

2) Ability to remove or change the function of the "Reactions" button.
This currently launches the transfusion reactions routine that the
pathologist uses to document follow-up of potential transfusion
reactions. NOT something we want our nurses accessing!

3) Next Due vital signs that displays on the status board is currently
calculated based on the time the last set was documented. So if the
nurse is ten minutes late documenting a set of vitals, the system will
show the next set as being due ten minutes later than it is actually
due. These times need to be calculated from the start time of
infusion... not the last time documented.

4) Vital Signs taken via the TAR (Transfusion Administration Routine)
are attached to any unit of blood currently infusing. But what about
baseline vitals taken immediately prior to starting or vitals taken one
hour after completion? We need a parameter before/after the start/stop
time of a blood product that would attach vitals that during that
defined period of time.

5) No "Next Vitals Due" if multiple units infusing. This is probably
not a huge deal... but it would be nice to see the next time vitals are
due for ANY unit infusing. Otherwise we are forcing nurses to manually
track when they need to do this when they have multiple units infusing.

Hope this helps,
Kenny Whiteside

We have been testing the transfusion record and have a number of
concerns.
Is anyone using the record in a live environment? We are Magic 5.5
SR3.
Thanks in advance.
